Nairobi City County Government spends millions of shillings on wages for locum doctors, while staff physicians manipulate the duty roasters to work in their private health facilities.
It has also emerged that chief executive officers heading the level four and five county hospitals in Nairobi City do not qualify to hold the positions because they were allegedly hired illegally.
An official in the health sector at City Hall who spoke on condition of anonymity, disclosed that staff doctors collude with chief officers to deploy locum doctors who cover-up for them, while they operate in private health facilities.
”The county government spends over Kshs.30 million monthly on locum doctors yet we have staff doctors who rarely report to work because they operate their own facilities by colluding with chief officers at City Hall”, said the official who cannot be quoted because he is not allowed to speak to the media.
This new development comes in the wake of a memo by the County Secretary and Head of County Public Service, Godfrey Akumali, on a decision by the County Executive to introduce new bank accounts for all hospitals and health facilities.
The memo dated November 5, 2025 is addressed to Chief Executive Officers of Level 4 and 5 county hospitals and the officers-in-charge of all other county health facilities.
Mr Akumali has instructed all the heads of county hospitals to facilitate the opening of bank accounts at Sidian Bank for their respective hospitals and health facilities.
Governor Johnson Sakaja is said to have allegedly appointed the Chief Executive Officers of Level 4 and 5 county hospitals illegally, without following due process.
Further, investigations reveal that the county government has no policy or regulations for the level four and five health facilities.
This, has resulted to reported mismanagement at Mama Lucy Kibaki and Mbagathi hospitals, by administrators who were appointed in contravention of requisite provisions.
In addition, the reporting structure of the CEOs does not conform to the pecking order in the county executive organogram.
Under the county administrative structure, level 4 and 5 hospital administrators are supposed to report to the health sector chief officer.
However, they were placed on ranks higher than the chief officers and therefore by-pass their sector heads to report directly to the County Executive Committee Member in charge of health services.
